This actually reminds me of a story--the last show I played with my crappy Marshall AVT150 head, it died on me two and a half songs into the show. Fortunately, my friend's Fender Bassman head was already set up on stage, so I plugged into that instead. We started the song again, and before long the amp died again. Getting more and more exasperated, I looked in the back of the amp, and one of the power tubes had just fallen out. My friend had a little whiskey bottle in there that he used to wedge the tube into place. We put it back and played the rest of the show, and later on I found him and mentioned that the tube fell out. He was just like "yeah, it does that sometimes". So sometimes they do just fall out.

Probably not in your amp, though--you'd know if they were loose.

That's right. They used to wiggle out on my friends Bassman too, and sometimes they do that on the Deluxe Reverb we have. It will make a crackely sound that nobody likes... not even the drummer.
